Due to strange circumstances of this year, we must have every student fill out a field trip form for our “off-campus” End of Year Concerts. You will only need to fill out one set of forms for ALL concerts – at Oak Bay (jazz ensembles, strings and band) and/or at St. Dunstan’s Church (all choirs). Because we are off site, we must ensure we have proper contact info for all students. We will give everyone a paper copy in class this week, but here are the forms if you misplace them. You need a parent or guardian to sign them, email them to Ms. Kerr email@example.com or Mr. Davis firstname.lastname@example.org (even if it is a picture of it), or hand them to us in person. These forms are due THIS FRIDAY, June 4th.
